Medication-related errors can occur anywhere in the prescribing process. Such errors include anything from a prescription with an unreadable handwriting, inaccurate transcriptions made at the pharmacy, to an inappropriate administration of the medication due to incorrect instructions given by medical assistants, nurses and other health professionals.
