Rewrite in slope intercept form:

x-4x=12

Y always has to be on the left side so that's what we have to do.

x-4y=12
divide everything by 4 so 4y become y:
1/4x-y=3
-1/4x from each side:
-y=-1/4x+3
switch signs on everything:
y=1/4x-3
