Errors in mefical reports can cause legal issuses, to insure your transcripts are accurate what would u do if you come across a blank section in a dictation

The best thing to do when you see a blank section is to ask your doctor or to ask the appropriate authority, how did this blank section arise and to try and get it fixed. That is the only and best way of perhaps getting it fixed. In any other case, there is a very slim chance that it will get fixed. 

They probably don't understand either 1. what the nightmare is about or 2. his anxiety and stress
