Reason for Visit: Routine gynecological examination with Pap smear. History of Present Illness: Patient had a hysterectomy five years ago, nonmalignant circumstances. No complaints. Medications: None; takes a vitamin every day. Examination: Generalpleasant, well-developed, well-nourished female. HEENT: Eyespupils equal, round, and reactive to light and accommodation. Nose and throatnormal; no redness or discharge. Neck: Supple, with no thyromegaly or tenderness. No lymphadenopathy. Respiratory: Lungs are clear to auscultation and percussion. Cardiovascular: RRR, no murmur. Breasts: Symmetrical, no masses on palpation, no discharge. Abdomen: No splenomegaly, soft, nontender. Normal bowel sounds. Pelvic: Absent uterus and cervix, no masses. Pap smear was completed. Rectal: Deferred. Extremities: No cyanosis or clubbing. No peripheral edema. Pedal pulses intact. Skin: Free of lesions. Neurological: Reflexes are symmetrical. Assessment: Normal gynecological annual exam, except for absence of uterus. Plan: Patient will be contacted on Pap smear results.
