Scenario:
You are employed as a safety officer in a construction company in Hong Kong. An
accident happened in one of the construction sites with the following details:
A 20-year-old apprentice carried out the work of grinding a small workpiece using
an abrasive wheel in the construction site. He wore the safety helmet and eye
protector for the grinding work. The floor of the workplace was filled with
construction debris.
Before the grinding work, he picked the wheel from the bench and mounted it
himself in the abrasive wheel. The spindle speed and the wheel speed were found
500r. p.m. and 400r. p.m. respectively. The gap between work-rest and wheel was
adjusted to 5mm. The workpiece was held with a plier. Accidentally the workpiece
was jammed between the abrasive wheel and the machine guard. Then the abrasive
wheel burst into pieces and splashed out. The apprentice was then seriously hurt.
